SSA form 7162
Have any of you from the USA received the "proof of life" form; SSA-7162... They're supposed to be sent out in May or June, but I haven't gotten one yet. I've changed my address, wondering if that's the problem.

What was the post mark date as wondering how long it actually took to get to you

No post mark, nor date. I would imagine it takes close to 2 weeks. However, in the past when I did not receive the form, I downloaded the form through the internet and then sent it in, as described above.

Received my form today. The date when your form was generated most likely is at top of form. It says Jun 6 on mind.

I didn't receive one this year, so I finally printed it off the internet and returned it: EMS with tracking was 1,550 baht...

Mailed mine registered air mail with tracking. 335 Baht. Tracking showed it taking 15 days for delivery. Tracking showed it was held up in U.S. customs for 9 days. Can't imagine why.
