Being reported on TV Channel News Asia now. His 13 year old step daughter and the elephant's handler were injured as well. The TV commentator said incidents like this are more common than reported. Surprise surprise. I had to do a search to find it in the Post:

The last time a took a ride on one he decided he wanted to take a rest and play as we crossed a river. He dropped down and rolled on his side, sending me flying :shock:

This story was covered by the BBC shortly after it happened ...very sad indeed .

Ive been on a jungle elephant trek in Northern Thailand , a full day , jungle camp overnight then another day back to base must say it was a fantastic experience ...i recall thinking at the time ..i know who the boss is in this outfit - the one with the big ears .
